How to Print Your PhilHealth MDR Online

1. Log in at https://memberinquiry.philhealth.gov.ph/member/
2. Open Member Data Record or MDR from the menu
3. Check your name, birth date, PIN, category and dependents
4. Click Print or Download
5. Save the PDF
Print on A4 or long bond paper if your employer or hospital asks for a paper copy.
6. Log out
